Dr. Mike as his patients call him, received a D.M.D. degree from the University of Lewisville, Kentucky in 1976. He then served as a dental officer in the U.S Navy Dental Corp for two years in San Diego California at the Navy Dental Center where he revived further training.

In 1978 he moved to Utah an opened a private practice in Cottonwood Heights. He taught as a clinical instructor at the University of Utah Medical Center in the graduate dental residency program for over a decade and ran a clinic for the department of health in Salt Lake City.

He has been practicing dentistry in Cottonwood Heights and South Jordan with an Emphasis on interceptive orthodontics, treatment of obstructive sleep apnea, sleep related breathing disorders in children, dental implantology and cosmetic dentistry. He is an accredited provider for The Healthy Start program to treat sleep disordered breathing in children and develop healthy normal dental arches.

He is a member of the American Academy of Dental Sleep Medicine, The American Academy of Implant Dentistry, The Utah Dental Association and is an instructor for the Academy of Minimally Invasive Implantology. Dr Van Leeuwen enjoys fishing for all types of fish, amateur radio (ham radio) and playing his musical keyboards.

He is a husband to his wife Connie and they have 1 daughter and 3 sons. He always makes time to go camping and be with his grandchildren.
